Welcome to the North Central Texas College Online Athletic Pre-Participation Forms Portal.

NCTC would like to thank you for taking part of our new process of accepting athletic pre-participation paperwork online. This new automated online process will allow us to be more efficient in handling athletic pre-participation paperwork as well as saving valuable resources.

NCTC Student-Athletes should have completed the following:

1.) The fully completed RankOne sport forms. Which reviewing and signing the NCTC Student Athlete Handbook.

2.) Download a copy of the Front and Back of your insurance card into RankOne Sports.

3.) Download and print the NJCAA Physical and Medical history. The required NJCAA physical and medical history cannot be filled out online because it must be completed by the appropriate medical professional. You may print a physical and medical history by selecting the physical and medical history tab at the top of the page. Once you have selected the tab you will see the link to the physical and medical history and will be able to download and print.

Rank One forms and NJCAA Physical and Medical history must be turned into the NCTC Athletic Training staff before you can participate in any sports related activity, including but not limited to; weight lifting, conditioning and practice.
